YOUR INFORMATION & THIS WEBSITE

Privacy Notice

This notice explains how information is handled when you visit this website or contact the practice.

Last updated: 27 October 2026

Who this website represents

Dr. J Yamamoto Functional Medicine is the independent professional identity of Dr. Joe Yamamoto and Dr. Joanna Yamamoto. This website shares their approach to care, educational resources, and information about clinical and professional contact pathways.

Responsible operator/controller: Dr. Joanna

This notice covers the public website and initial correspondence. Detailed health information collected for assessment and care requires privacy information appropriate to the intake and clinical-care process.

Information handled through each pathway

Visiting the website

The website hosting service processes technical access information to deliver and protect the site. OpenAI Sites also provides site traffic statistics, including visitor and page-view counts. We have not added advertising pixels, a newsletter sign-up, public comments, or a patient-account system to this website.

Email inquiries and appointment requests

If you email us, the correspondence may include your name, email address, message, and the details you choose to share. These are used to respond to the inquiry, discuss an appointment request, or coordinate relevant professional correspondence. Please keep the first message focused on the inquiry and avoid sending medical reports or detailed health histories by general inquiry email.

Patient intake and clinical information

Patients will be asked to complete an intake form during appointment scheduling. The form is supplied privately after appointment confirmation and is not linked from this website. This website does not collect or store intake responses in its content-management database. Review the collection and privacy information supplied with the form before submitting health information.

Private content workspace

The owner-only editor uses ChatGPT sign-in to establish identity and restrict editing. It stores website drafts, published content, revision history, and the editor’s account identifier for change records. It is not a patient portal or clinical record system.

Purpose and basis for processing

Initial inquiry emails are used to answer inquiries and coordinate appointments. Website technical information supports website delivery and security, while editor information supports maintaining website content. The relevant basis depends on the activity, such as responding to steps you request before arranging a service, applicable legal obligations, or consent where required.

The inquiry process described in this notice does not include newsletters or promotional mailing lists. Any future newsletter or promotional use will be addressed separately, with updated information and an appropriate basis before that use begins. Health-information collection and clinical-care processing need their own appropriate basis and collection information.

Service providers and access

Website hosting: OpenAI Sites and its infrastructure, including Cloudflare.

Email service: Google (Gmail)

OpenAI’s privacy information is available at openai.com. The email service supplies its own applicable privacy information. The privately supplied intake form will include information about the service used and its information handling.

Inquiry correspondence may be accessed by Dr. Joanna, the assistant, and relevant members of the medical team involved in the patient’s care, as needed to answer inquiries and coordinate appointments. Service providers may process information as described below. Processing through online providers may involve systems outside the Philippines.

Unpublished drafts and routine inquiry correspondence are not intended for public disclosure. Clinical information should be provided through the designated care process, where the responsible physician or facility and any relevant sharing are identified.

Storage and retention

Routine inquiries that do not lead to an appointment are retained for six months after the last substantive exchange and deleted at the next monthly review. Dr. Joanna is responsible for reviewing and deleting this correspondence. Active scheduling and unresolved inquiries are reviewed monthly and retained while needed to address them.

When an inquiry leads to an appointment, information necessary for care is handled through the designated patient-record process under the applicable clinical-record retention policy. Necessary correspondence relating to a complaint, dispute, or legal obligation may be retained while that reason applies, with the reason documented and reviewed periodically. Spam and irrelevant messages are deleted promptly.

Deletion includes eligible email threads and attachments, unnecessary downloaded or forwarded copies under the practice’s control, and permanent deletion from Gmail Trash. A brief review log records the review date, reviewer, number of threads deleted, and any retention exceptions without copying patient details. Privacy deletion requests are reviewed individually by Dr. Joanna rather than waiting for the monthly cycle. Hosting and email providers may retain technical logs or backups under their own applicable arrangements.

Clinical records and intake submissions may have separate retention requirements. Hosting and other providers may also retain technical logs or backups under their own applicable arrangements. Retention information for those services should be considered with the relevant collection notice.

Security and choices

The website uses HTTPS, restricts editing to the authorized owner, and separates saved drafts from published content. These measures do not make general email an appropriate channel for sending detailed health records.

Email links open your email service outside this website. That service may use its own cookies or similar technologies. The privately supplied intake form has its own applicable collection and privacy information. ChatGPT sign-in for the private editor is handled by the hosting platform; public visitors do not need an editor account.

Your privacy rights and contact

Dr. Joanna handles privacy questions and requests sent to the privacy contact email below. Subject to applicable law, you may ask about information held about you, request access or correction, object to processing or withdraw consent where applicable, and request erasure or blocking where the conditions for doing so are met. Some information may need to be retained for lawful purposes. Identity verification may be needed before a request can be acted on.

For privacy questions or requests, email inquiry.drjyamamoto@gmail.com. Please identify the request without attaching unnecessary health records or identification documents to the initial email.

You may also raise a concern with the Philippine National Privacy Commission. Information about data-subject rights and complaints is available on its official website.
